How can I change the registration form

I have tried to change the wizard box signup.ptd twice now, but when it comes to testing it, I get the error invalid form token. All I need on my registration form is :

• First Name
• Last Name 
• Country
• Email
• Username

I do not need tax numbers or Company names or anything like that. Please could someone maybe attach a signup.ptd with those options for the wizard box order form.
